Bachelor's, Master's, Doctorate, or Certificate degree in Physical Therapy Minimum 1 year of experience required Active Physical Therapy License required if applicable Current BLS certification (AHA or Red Cross) required prior to start date Primary source verification of professional license and/or certification required within 30 days of start DOB and SSN required for consideration Additional Information
