Provide direct physical therapy services to patients in an outpatient clinic setting Evaluate, assess, and develop individualized treatment plans for patients Deliver hands-on therapeutic interventions in alignment with plan of care goals Complete required clinical documentation accurately and in a timely manner Collaborate with interdisciplinary team members to support patient outcomes Required Qualifications
Active Physical Therapist license in the state of Arizona (permanent license required) Prior experience in a physical therapy outpatient or clinic setting Ability to meet facility-specific credentialing and documentation requirements prior to start Candidates who have worked permanently or PRN at this facility within the last year are not eligible for this assignment Preferred Experience / Skills
Experience working in a high-volume outpatient physical therapy environment Strong documentation skills and familiarity with electronic medical records Why Join Us
