Active Physical Therapist license in the state of Arizona (permanent license required) Prior experience in an outpatient physical therapy setting Ability to meet credentialing requirements prior to assignment start Must provide proof of license application within 5 business days of booking if licensure is pending Preferred Experience / Skills
