Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting Develop and implement individualized physical therapy treatment plans Monitor and document patient progress toward established therapy goals Collaborate with interdisciplinary care team to coordinate patient care Educate patients and caregivers on therapeutic exercises and home programs Required Qualifications
Active Physical Therapist (PT) license in the state of Oregon or ability to obtain prior to start Graduate of an accredited Physical Therapy program Current BLS/CPR certification Experience working in a SNF or post-acute care setting Preferred Experience / Skills
Prior travel or contract therapy experience Familiarity with skilled nursing documentation and Medicare guidelines Why Join Us
