Post-acute care setting within a home health & hospice therapy unit Day shift schedule, 8:00 AM to 4:30 PM No weekend shifts required 40 hours per week guaranteed 13-week assignment length Uses Point Click Care (PCC) for charting Requirements
Minimum 1 year of relevant Physical Therapy experience required Current BLS certification required Active PT license required (if applicable) Valid driver's license required for consideration Two professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and rehire eligibility Additional Information
Candidates residing within an 80-mile radius of the facility may be considered as local candidates at a lower pay rate First-time travelers may be considered for this position
