American Traveler is seeking a licensed Physical Therapist with a minimum of 2 years of SNF/ICF experience for a 13-week long-term care assignment in Hawaii. Details
Work setting is a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) serving veterans Day shift position with a 5-day work week; departmental coverage spans 7 days, so schedule will likely include one weekend day Floating to other units or facilities may be required based on census and staffing needs EMR systems include Net Health Rehab Optima and Epic Requirements
Minimum 2 years of experience in a SNF/ICF setting preferred Active State of Hawaii PT license in good standing required by start of assignment; license required if applicable Current BLS certification required Additional Information
Responsibilities include performing evaluations, reassessments, treatments, and clinical services consistent with licensure and scope of practice Develops and implements individualized treatment plans in collaboration with physicians and the interdisciplinary team Educates residents, families, and caregivers regarding treatment plans and patient progress Completes accurate and timely documentation in accordance with facility, state, federal, and payer requirements May supervise therapy assistants as applicable to role Must be available to work Hawaii State and Federal holidays as needed based on facility operations Orientation provided for the rehabilitation department, EMR documentation, and facility workflows Candidates must arrive the Thursday prior to the assignment start date for a required employee health appointment on Friday
