2+ years of current home health experience
2+ years of current occupational therapy experience
OASIS experience
Proof of auto insurance and a clean driving record
Active American Heart Association Basic Life Support (BLS) certification
Verified work history and skills relevant to occupational therapy and home health, current within the past year
Two supervisory reference forms completed within the last two years
Perm address must be included in the profile
All scheduling requests must be made at the time of submission; no requests can be accommodated after offer or interview

Additional Information:
No holiday or on-call coverage required
Dress code: Olive green scrubs
Typical daily mileage: approximately 35-50 miles, paid at current Federal Rate for travel to and from the main office
No RTO will be accepted for this assignment; candidates residing within 50 miles of the facility will not be considered
