Travel Job Summary:
Occupational Therapist (OT) specializing in Home Health services in Arbor Vitae, Wisconsin. This position involves providing home health therapy to geriatric patients across territories mainly covering the Oneida and Vilas County areas. The role requires timely documentation, communication, and flexibility with territory coverage.
Job Details:
Schedule: Days shift, with an 8-hour shift, minimum of 40 hours per week.
Required Qualifications: One (1) year of relevant experience, BLS certification, active drivers license, proficiency in HCHB (Homecare Homebase), OASIS experience required for staff completing Start of Care (SOC), recent six visits daily, at least 30 points weekly for productivity, and recent hospital or community home health experience preferred.
Preferred Qualifications: Experience with geriatric populations, home health care, and the ability to work independently within territory.
Additional Information:
Patient visits average 5-7 per day.
No on-call, weekends, or first-time traveler restrictions.
The position requires flexibility with territory within a 60-mile radius, timely reporting, and documentation.
Orientation begins on Monday.
Travel pairs accepted but each will have own territory and transportation.
The role involves working in a home health environment with applicable equipment and procedures.
Mileage reimbursed at IRS standard rate.
Must have an active driver's license and personal transportation.
