Responsibilities
Case Manager RN – Days – 40 hours per week, no weekends.
This role oversees guardianships, complex medical and placement services, and social situations.
Average daily census: 15 cases daily.
Patient diagnosis: inpatient case management providing and coordinating medical home care, placement, and social issue wrap‑around type services for safe discharges.
Qualifications
Minimum 3 years of experience; first‑time traveler with inpatient hospital/acute care experience is acceptable.
EPIC charting preferred.
RN license required (MD state). BLS preferred.
Shift/hours: 5 days × 8 hours, Monday through Friday block (08:00 or 08:30‑16:00 or 16:30 depending on needs).
