Responsibilities

• Provide comprehensive, holistic care to seniors in community settings.
• Conduct patient assessments, develop care plans, and manage treatment.
• Coordinate interdisciplinary care and facilitate home visits.
• Maintain accurate medical records and comply with care protocols.
• Participate in on‑call rotation as needed.
• Engage with Medicare/Medicaid program requirements.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant.
• At least two years of experience working with the elderly.
• Prescriptive authority and DEA registration.
• Eligible to serve Medicare/Medi‑Cal participants.
• Must obtain CPR and First Aid certification within 180 days of hire (certification upon hire preferred).
• Role requires travel throughout California clinics.

• Competitive compensation: $171,697 annually + quarterly bonus.

Compensation

$168,895–$232,231 USD per year + bonus. Salary is adjusted based on experience and other qualifications.
