Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You will travel across a defined coverage area, perform 45-minute visits, and use an iPad-based workflow to document health data.
Must hold board certification and be licensed, with multi-state licensure as needed. Ideal candidates are flexible for local travel, perform physical evaluations, and may conduct virtual visits when appropriate.
