Travel/Local Contract – LTC Licensed Practical Nurse (LPN)

13-week assignment in or near Warren, Minnesota. Provide direct patient care in a long-term care setting, collaborating with interdisciplinary teams, and maintaining accurate documentation.

Key Responsibilities:

• Provide direct patient care in accordance with LPN scope of practice, including medication administration and routine assessments
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Maintain accurate and detailed documentation of patient conditions, treatments, and interventions within EMR systems
• Educate patients and their families on health management, medications, and care procedures
• Ensure compliance with all facility policies, safety protocols, and state regulations during shift operations

Qualifications:

• Completion of an approved Practical Nurse training program from an accredited institution
• Valid Licensed Practical Nurse (LPN) license in Minnesota
• CPR certification
• At least 1 year of recent experience working in a Long Term Care setting
• Familiarity with electronic medical record (EMR) systems
• Ability to work evening and night shifts
• Excellent communication and interpersonal skills
• Ability to stand for extended periods
• Reliable transportation
• Strong attention to detail

Preferred Skills:

• Infection Control certification (preferred)
• Experience working evening or night shifts (preferred)
