Current, active Occupational Therapist license required, if applicable Minimum 1 year of relevant experience required No employment gaps of 90 days or more within the past 12 months Additional Information
Candidates must reside outside a 50-mile radius of the facility to be considered US Social Security Number and date of birth required for consideration; Canadian SSNs are not accepted
