- Schedule: 4 days per week, 10 hours per day, with a total of 40 hours per week n
- Required Qualifications: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BSPharmacy) degree, valid pharmacy license in the state, BLS or ACLS certification (AHA only), minimum 0 years of experience, location within the service region, and experience with Epic electronic health record system is highly preferred n
- Responsibilities: Prepare and dispense drug orders according to physician requests; interpret verbal and written medication orders; maintain accurate patient medication records; compound and dispense pharmaceuticals, including sterile and non-sterile products; issue controlled substances in compliance with regulations; provide clinical pharmacy services such as medication management and drug therapy monitoring; participate in interdisciplinary rounds and quality improvement activities; supervise pharmacy support staff; ensure licensure and Continuing Education documentation are current; perform clinical interventions including pharmacokinetic dosing and medication reconciliation; participate in antibiotic and opioid stewardship programs and short-term medication management activities; ensure safe, appropriate, and cost-effective drug therapies. n
