American Traveler is looking for a Physical Therapist for a Home Health position requiring 1+ year of home health experience and knowledge of HCHB and OASIS. Details
Home health setting providing physical therapy services in patients' homes Day shift schedule, 5x8s, Monday through Friday with no weekends No call requirements Mileage reimbursed at government rate Must be proficient with HCHB and OASIS documentation systems Requirements
Doctor of Physical Therapy (DPT) degree required if licensure obtained on or after January 2016, or foreign equivalent Current PT licensure required in state of practice; license required if applicable BLS or CPR certification required from an approved vendor Valid driver's license required, must be at least 21 years of age Minimum 1 year of home health experience required No prior adverse actions on license permitted Additional Information
Conducts evaluations and establishes individualized treatment and discharge plans for patients Provides therapy interventions using appropriate clinical techniques and protocols Serves as co-leader in an interdisciplinary care team approach Supervises assigned physical therapy assistants Completes comprehensive and accurate clinical documentation within established deadlines Located in Lake County, Northern California, near Clear Lake and within two hours of the San Francisco Bay Area All required vaccinations must be maintained as a condition of employment, with medical and religious exemptions considered Accepts local candidates with no radius rule, local rates match travel rates Candidates with more than 5 days of requested time off will not be considered
