Location: Montrose, NY 10548 Full-time, onsite position Schedule: 40 hours/week, 7:00 AM–4:00 PM Rotating weekends and holidays Current New York State PT or OT license required Minimum 5 years of licensed therapist experience in an SNF rehabilitation department Previous rehabilitation leadership and staff supervision experience required Strong experience with quality assurance, clinical documentation, and regulatory compliance Knowledge of geriatric rehabilitation, long-term care, and resident safety Responsible for therapy department operations, staffing, scheduling, and program development Provide direct PT/OT services within NYS license Coordinate care plans, discharge planning, equipment needs, and interdisciplinary communication Support staff recruitment, orientation, competency validation, and in-service education Facility credentialing, physical exam, and TB/PPD documentation required before start Must maintain timely and accurate EHR documentation and resident confidentiality
