Job Opportunity – Registered Nurse, Case Management – Apple Valley, CA 92307

Start Date: August 31, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Patient ratios: 1:25 on weekdays, 1:45 on weekends. Every other weekend rotation required to meet department needs.

Highlights:

• 13-week assignment at an acute care facility
• Collaborative interdisciplinary team including Physical Therapy and Social Services
• Diverse patient population including neonates, adolescents, adults, and geriatrics
• 40 hours of orientation provided
• Business attire or scrubs accepted
• Up to 7 days of requested time off available

Requirements:

• Active California RN license (pending license accepted but must be active by start date)
• Minimum 2 years of acute care Case Management or Discharge Planning experience in a hospital setting
• Previous travel nursing experience required; first-time travelers cannot be considered
• Epic charting system experience required
• Expertise in assessing patients, care coordination, discharge planning, and utilization review
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, advance directives, POLST, and protective services processes
• Experience with family meetings and treatment plan development
• Proficiency with computerized documentation systems
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
