Description
American Traveler is seeking an experienced RN Case Manager for an acute care hospital assignment requiring a CA RN license and 2+ years of acute care case management/discharge planning experience.
Details
- Acute care hospital setting within the Case Management department
- Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
- 13-week assignment length
- Patient ratios of 1:25 on weekdays and 1:45 on weekends
- Every other weekend required to meet department needs
- No floating expected
- Uses Epic charting system
Requirements
- Active CA RN license required, pending license accepted if licensed by start date
- BLS certification required
- Minimum 2 years of acute care case management/discharge planning experience required
- Hospital experience required; behavioral health experience does not qualify
- Prior travel nursing experience required, must be a seasoned traveler
- Epic EMR experience required
- 2 supervisory references covering 1 year of employment within the last 3 years
Additional Information
- Responsibilities include care coordination, discharge planning, needs assessment, DME ordering, plan of care management, and prior authorizations
- Familiarity with CMS, Department of Health, DRG, and Joint Commission/Core Measure/National Safety Goals regulations is relevant to this role
- Business attire or scrubs accepted
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO limited to no more than 7 days
- Local candidates accepted within a 50-mile radius at a lower bill rate
- Former permanent staff must wait at least 3 months before returning as a traveler and must live more than 50 miles from the facility
- First-time travelers will not be considered for this position
