Patient Care Coordinator for pediatric patients for the Pediatric Complex Care Program
Duties Includes: patient education, reviewing after visit summaries, manage EPIC in basket, triaging calls, reviewing charts.
Will Manage EPIC Inbasket, triage calls, return messages to families, write letters , outreach to patients for barriers to care, work with DME, Nursing agencies, and other vendors and review charts. Will not need to do med reconciliation or refills
