This RN leads coordination of the pre-transplant journey, including referral intake, comprehensive clinical assessment, diagnostic and consult scheduling, and ongoing patient and family education. Collaborating with surgeons, gastroenterologists, and multidisciplinary teams, the coordinator develops individualized care plans, manages clinical triage, monitors labs and testing, and ensures regulatory and documentation compliance.
Role includes patient advocacy, transition planning, and participation in quality and outcomes initiatives within a mission-driven academic health system.
