To support the development of a comprehensive Transitions of Care program, the full-time Advanced Practice Provider will provide virtual clinical care post-discharge, conducting assessments, developing care plans, and collaborating with an interdisciplinary team to prevent patient readmissions. Key responsibilities Perform initial and acute symptom TOC visits, including comprehensive assessments and medication reconciliations Participate in interdisciplinary rounds to discuss high-risk patients and enhance team communication Engage in quality improvement projects and adapt to evolving healthcare needs and practices Required qualifications Minimum of 3 years of experience in hospital, post-hospital, or primary care settings Active RN and APRN or Physician Assistant licenses in good standing Ability to work full-time, including one weekend shift per month Active BLS certification at the time of hire Willingness to obtain licensure in required states within 90 days of hire
