Transitional Care NP/PA: Impactful, Team-Based Care

One Health is seeking a Nurse Practitioner or Physician Assistant to join the Transitional Care Management team. This role focuses on caring for high-risk patients recently discharged from the hospital, aiming to improve outcomes and reduce readmissions.

The candidate will collaborate with hospitalists, ED providers, and case management to ensure a smooth transition back to the community, within an evidence-based, patient-centered care model.
