Transition Specialist
Make a Difference in the Lives of Youth
We are seeking a dedicated and motivated Transition Specialist to help youth successfully transition back into their communities, schools, and career pathways. This position plays a key role in supporting young people as they work toward positive educational, employment, and life outcomes.
Responsibilities
  • Facilitate life skills and personal development programming.
  • Support youth in developing and achieving education, career, and transition goals.
  • Review transition plans and monitor progress toward successful reentry.
  • Participate in multidisciplinary team meetings and release planning activities.
  • Conduct follow-up contacts with youth after release to provide support and resource referrals.
  • Coordinate services with schools, employers, training programs, community organizations, and other stakeholders.
  • Assist youth with obtaining essential documents, including identification cards, birth certificates, Social Security cards, and driver's licenses.
  • Support enrollment or re-enrollment in education and training programs.
  • Provide information on employment opportunities, apprenticeships, vocational training, and career pathways.
  • Maintain accurate records, documentation, and case notes.
  • Represent the organization at meetings, conferences, and community events.
Qualifications
  • Bachelor's degree in Human Services, Social Work, Criminal Justice, Education, Psychology, or a related field preferred.
  • Experience working with youth, case management, education, workforce development, social services, or related areas preferred.
  • Strong communication, organizational, and relationship-building skills.
  • Ability to work independently and collaboratively with community partners.
  • Proficiency with Microsoft Office and basic data entry systems.

Pay: $21.00 per hour
