This clinician provides telehealth follow-up visits for older adults needing psychiatric medication management, with a focus on depression, anxiety, dementia-related concerns, cognitive impairment, and other medication-related issues. Care also includes cognitive screening, initial care coordination planning, and ongoing monitoring for medically complex geriatric patients across assisted living, long-term care, and community settings. The role is supported by nursing staff and supervisory psychiatric collaboration within a team-based, remote practice model.
Key Responsibilities
- Conduct telehealth follow-up visits for geriatric psychiatric medication management.
- Monitor treatment response and adjust medications with attention to polypharmacy, fall risk, and medical comorbidities.
- Perform cognitive screenings and assessments using tools such as MoCA, MMSE, or similar instruments.
- Develop initial care coordination and behavioral support plans.
- Collaborate with nursing staff providing ongoing monitoring and patient support.
- Communicate with the supervising psychiatrist as needed.
- Maintain timely and accurate documentation in the EMR.
Required
- Current Florida license and certification as a Physician Assistant or Psychiatric Mental Health Nurse Practitioner.
- Experience providing care via telehealth platforms.
- Strong clinical judgment related to medication safety in older adults.
- Ability to work collaboratively in a team-based care model.
Preferred
- Experience in geriatric psychiatry, long-term care, or dementia care.
- Experience with cognitive assessment tools and behavioral management strategies.
- Experience working with older adults or in geriatric mental health.
- Bilingual in English and Spanish.
- Established relationships within the local community.
