Job Summary:
Seeking a licensed Physical Therapist to provide comprehensive rehabilitative services in a skilled nursing facility. The therapist will evaluate patients, develop treatment plans, and deliver personalized care to support recovery and mobility goals.
Responsibilities:

• Assess patient conditions and establish individualized treatment plans.
• Deliver therapeutic interventions to improve mobility, strength, and functionality.
• Collaborate with nursing and healthcare teams to coordinate care.
• Document patient progress and adjust treatments as needed.
• Educate patients and families on therapeutic exercises and care plans.

Qualifications:

• Hold a current and valid Physical Therapist license.
• Demonstrate experience in skilled nursing or long-term care settings.
• Possess strong assessment and communication skills.
• Be able to work a flexible schedule including weekend rotations.
• Follow all applicable state and federal licensing requirements.
