n Job Summary:n
n Seeking a licensed Physical Therapist to provide outpatient home health services, including patient assessments and therapy plans in patients' homes or care facilities. The role involves traveling within a designated territory to deliver quality care and support patient recovery.n

n Responsibilities:n

• n Evaluate patient conditions through comprehensive assessments and develop personalized treatment plans.n
• n Deliver skilled therapy interventions to improve patient mobility and functional abilities.n
• n Document patient progress and maintain accurate records within required timeframes using EMR systems.n
• n Collaborate with patients, families, and healthcare teams to optimize care outcomes.n
• n Travel up to 75 miles to patient homes and facilities, ensuring timely appointments.n
• n Adhere to all compliance standards, including documentation and HIPAA regulations.n

n Qualifications:n

• n Hold a valid state licensure as a Physical Therapist.n
• n Have a minimum of 1 year of recent adult home health experience.n
• n Possess current CPR and BLS certification.n
• n Demonstrate proficiency in EMR systems such as HomeCare HomeBase, Oasis, or Outlook.n
• n Be willing and able to travel throughout the assigned territory.n
• n Must have reliable transportation, a valid driver’s license, and auto insurance.n
