We are seeking a licensed or license-eligible Therapist to join our Youth Assertive Community Treatment (ACT) team. This community-based program serves youth ages 10-21 with serious behavioral health needs, helping them remain safely in their homes and communities through intensive, team-based support.
Responsibilities:
- Operate within program policies, procedures, and agency practice guidelines.
- Adhere to state regulations and applicable codes of conduct.
- Collaborate within an interdisciplinary team to support client functioning and goal attainment.
- Provide person-centered, individualized family, individual, and group therapy in home, community, and office settings.
- Deliver services in a trauma-informed, culturally, and linguistically competent manner.
- Conduct comprehensive clinical assessments and develop individualized treatment plans.
- Apply agency-approved evidence-based interventions.
- Participate in daily team meetings, family meetings, supervision, and required trainings.
- Coordinate care with schools, DSS, and other collateral contacts.
- Provide crisis intervention and participate in an on-call rotation outside normal business hours.
- Assist with client transportation to service locations as needed.
- Complete timely documentation, reports, and other program-related tasks
- 60% Direct client contact (including transportation).
- 20% Documentation and case-related meetings.
- 20% Team meetings, supervision, and training.
- LMSW, LCSW, LMHC, or LMFT (limited permits accepted).
- Experience working with children/adolescents and families preferred.
- Strong documentation and computer skills (EHR experience a plus).
- Valid NYS driver's license and daily access to a personal vehicle.
