Providing support and guidance to members with chronic conditions, the full-time Texas Licensed Wellness Coach will conduct health assessments, implement care plans, and offer telephonic coaching to improve health and wellness outcomes in a remote setting. Key responsibilities Utilize clinical expertise to provide health support and solutions for members with chronic health conditions Perform telephonic assessments to evaluate member needs and readiness to change Engage members in developing wellness goals and monitor their progress to ensure effective care plans Required qualifications Associate's degree or higher; Master's degree in Mental Health, Counseling, Social Work, or Nursing preferred 2-4 years of related behavioral health or psychiatric experience Current Texas state licensure as an LPC, LCSW, LMSW, LMFT, or RN required Experience in psychiatric intensive outpatient programs or managed care behavioral health management Proficient in Microsoft Office applications; bilingual in English and Spanish is a plus
