Texas Licensed Transitions of Care APP

To support a growing Transitions of Care program, the full-time Bilingual (Spanish/English) Transitions of Care Advanced Practice Provider will provide virtual clinical care post-discharge, conducting assessments, developing care plans, and collaborating with an interdisciplinary team to prevent patient readmissions. Key responsibilities Perform initial and acute symptom TOC visits, conducting comprehensive assessments and managing patient care Participate in interdisciplinary TOC rounds to discuss high-risk patients and enhance team communication Engage in quality improvement projects and provide clinical education to the team Required qualifications At least 3 years of experience in a hospital, post-hospital, or primary care setting Active RN and APRN licenses or Physician Assistant license in good standing Ability to work full-time, including one weekend shift per month Active BLS certification at the time of hire Willingness to obtain licensure in required states within 90 days of hire
