Key responsibilities Monitor assigned phone lines and triage symptom-based calls according to protocols Document care recommendations in real-time and coordinate with local operations and acute care settings Develop and update care plans, ensuring compliance with state and payer requirements Required qualifications Degree/certification from an accredited school of Nursing Current licensure as a LPN in Texas, with a compact license in good standing At least one year of nursing experience, preferably in home health Clinical experience with individuals with intellectual and developmental disabilities or related disorders One year of home health experience preferred
