Leading clinical care delivery, the remote Texas Licensed Palliative Care Provider will coordinate services, support quality improvement initiatives, and collaborate with various teams to ensure high-quality supportive care for patients and their families. Key responsibilities Lead internal and joint case rounds to eliminate barriers to care and ensure appropriate care plan execution Coordinate with Network Providers and Longitudinal Care Providers for optimal service delivery Monitor and analyze network data to identify trends and implement necessary interventions Required qualifications Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies Five years of clinical experience in palliative care, hospice, or with patients with serious illnesses Active Registered Nurse or Physician Assistant license in Texas without board actions Board certification as an advanced practice nurse by a nationally recognized certifying agency Active Unrestricted DEA license
