To support a new Longevity Program, the remote Texas Licensed Geriatric Nurse Practitioner will deliver initial assessments and ongoing medical follow-ups to older adults via telehealth, focusing on comprehensive geriatric assessment, chronic disease management, and cognitive health. Key responsibilities Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk Provide medical follow-up visits for chronic disease management and ongoing care Perform cognitive screening and staging, escalating for further assessment as needed Required qualifications Active, unrestricted NP license in Texas Board certification as an Adult-Gerontology NP or Family NP with a geriatric focus Active DEA registration Experience with Medicare documentation and E/M coding Comfort with telehealth and EHR documentation
