Collaborating with an interdisciplinary clinical team, the part-time Licensed Clinical Social Worker (LCSW) will deliver comprehensive, patient-centered palliative care remotely to individuals with serious illness, focusing on psychosocial support, care coordination, and advance care planning. Key responsibilities Conduct comprehensive assessments to evaluate psychosocial distress, mental health needs, and social determinants of health Facilitate serious illness conversations and advance care planning with patients and families Provide clinical recommendations and support care coordination within the interdisciplinary team Required qualifications Master's in Social Work (MSW) from an accredited program Active, unrestricted Licensed Clinical Social Worker (LCSW) license in Texas Clinical experience in palliative care, including advance care planning Experience working within an interdisciplinary care model Proficiency in using electronic health records (EHRs) and clinical documentation systems
