To provide high-quality, cost-effective healthcare outcomes, the full-time Texas Licensed Care Manager (RN) will develop and facilitate complex care management activities remotely, focusing on personalized care plans and education for members and their families. Key responsibilities Evaluate member needs and barriers to care, recommending plans for optimal outcomes Develop ongoing care plans and collaborate with providers to address unmet needs Monitor member status and adjust care plans as necessary to meet goals Required qualifications Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing 2 - 4 years of related experience, preferably in bedside NICU RN - Registered Nurse - State Licensure and/or Compact State Licensure required
