Leading a care management team, the full-time Texas Licensed Care Management Coordinator will perform outreach, manage inbound calls, and schedule services while serving as a point of contact for members and providers to resolve care plan-related inquiries in a remote setting. Key responsibilities Guide member outreach and support with care plan next steps and resource connections Serve as front-line support for complex member and provider inquiries, explaining care plan procedures Document and maintain non-clinical member records in compliance with state and regulatory requirements Required qualifications High School diploma or GED with 4+ years of related experience 2+ years of care coordination experience with diverse populations Experience with Medicaid and/or Medicare programs Strong documentation skills with experience in TruCare Cloud Proficiency in Microsoft Office Suite, including Excel and Outlook
