To support a growing care coordination team, the full-time Remote Care Coordinator Nurse will provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible members, ensuring they access appropriate care while working from a dedicated home office in Texas or select states. Key Responsibilities Conduct outreach to educate members about program benefits and facilitate their engagement in care Assess members' needs and develop personalized care plans to address their health goals Manage a caseload, coordinating care with providers and ensuring timely communication and documentation Required Qualifications Active and unrestricted LVN/LPN or RN license 2+ years of experience as a care coordinator, case manager, or related role 1+ years of remote work experience Experience working with Medicaid and/or Dual Eligible patients Must reside in Texas or select states including Louisiana, Mississippi, Arkansas, Missouri, Oklahoma, Kansas, Utah, Nebraska, or Wyoming
