To support a growing care navigation program, the full-time Remote Texas Licensed Care Concierge will manage a caseload of 75-150 patients, providing proactive care coordination, education, and advocacy for older adults with serious chronic conditions while working remotely. Key Responsibilities Serve as the primary point of contact for enrolled patients, conducting regular check-ins to assess health status and care needs Navigate patients through the healthcare system by coordinating appointments and facilitating communication between providers Conduct structured screenings for social determinants of health and connect patients with appropriate community resources Required Qualifications Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential 1+ years of experience in remote care coordination or similar patient-facing roles Experience working with Medicare-enrolled or dual-eligible populations Strong understanding of care coordination principles and healthcare systems High school diploma or equivalent required; associate's or bachelor's degree in healthcare or related field preferred
