We’re seeking a Territory Operations Director – RN to oversee clinical and operational performance across the Lombard, IL market . This is a high-impact leadership opportunity for someone who can combine clinical expertise, people leadership, operational strategy, and business acumen to create exceptional outcomes for patients, families, and employees.
We’re open to experienced leaders coming from a variety of healthcare leadership backgrounds—including Home Health, Hospice, Patient Care Management, Clinical Management, Branch Management, and similar roles .
In this role, you’ll guide daily territory operations and clinical services while supporting and developing a strong team of healthcare professionals.

This is a hands-on leadership role for someone who enjoys being visible, building relationships, solving problems, developing people, and improving healthcare operations.
- Guide daily operations and clinical services to best support patients, families, and team members
- Organize and lead agency functions and day-to-day territory operations, including oversight of multiple sites when needed
- Drive strong operational performance aligned with established Administrator KPIs
- Support, empower, and develop direct and indirect team members across the territory
- Provide administrative oversight to ensure delivery of high-quality, compassionate care
- Ensure policies and procedures are consistently implemented in accordance with professional standards and state and federal requirements
- Lead the selection, onboarding, coaching, and ongoing development of team members
- Build capable and confident leaders throughout the organization
- Conduct regular coaching conversations with direct reports to support growth and development
- Proactively identify retention risks and develop action plans to improve employee engagement and retention
- Successfully navigate a matrixed healthcare environment through collaboration and strong cross-functional relationships
- Partner with leadership and other departments to achieve shared goals and deliver meaningful results
- Minimum 2 years of healthcare/health-related experience
- Bachelor’s degree from an accredited college or university
- 3+ years of people management and administrative leadership experience
- Previous Home Health or Hospice experience preferred
- Candidates with leadership experience in roles such as Patient Care Manager, Clinical Manager, Branch Manager, Clinical Director, Administrator, Director of Nursing, or similar healthcare leadership positions will be considered
- Experience managing a large branch or patient census is preferred, particularly 400+ patients
- Candidates with smaller census experience may also be considered if they demonstrate strong leadership, operational, and team-management experience
- Minimum 2 years of experience with business and fiscal functions in a healthcare setting
- Strong business and financial acumen, with P&L oversight experience preferred
- Strong understanding of clinical operations, staffing, compliance, quality, and healthcare performance
- Proven ability to coach, develop, engage, and retain healthcare professionals
- Strong communication, organizational, problem-solving, and relationship‑building skills
- Valid driver’s license and reliable, insured automobile meeting state and organizational requirements

$130,000–$145,000/year based on experience
