To support a growing practice, the PRN Social Worker will provide high-quality therapeutic services to older adult patients in a fully remote environment, collaborating with an interdisciplinary team and participating in community education initiatives. Key responsibilities Collaborate with a small interdisciplinary team to deliver therapeutic services, including group, individual, and family therapy sessions Participate in community education efforts to promote the importance of behavioral health among local medical facilities and community groups Facilitate treatment team meetings and ensure compliant documentation and patient-centered treatment plans are developed and updated Required qualifications Tennessee Licensed Social Worker or Counselor (LISW, LCSW, LPC, LMFT, etc.) - required Commitment to working with kindness and accountability to enhance patient quality of life - required
