JOB COMMENTS/REQUIREMENTS: 8a-4p. Pewter scrubs. Looking for skilled OT for POD6 area of Banner Home care, mostly Maricopa, Casa Grande, Eloy coverage with flexibility to travel to other areas within East Valley Team.AZ RESIDENTS ARE NOT ELIGIBLE FOR ASSIGNMENTS IN ANY ARIZONA

Candidates
must be able to start on the listed start date, or no more than one week later
•
RTO must be on the cover page and cannot exceed 7 days. No RTO
during the first week of assignment
•
Please list all facilities the candidate is willing to work in. Specify
facility name(s), states, or all, if open to any facility in any state
•
All shifts the candidate is willing to work
•
All departments the candidate is experienced and willing to work

RequirementDescription:

Must have a minimum of 24 months OT experience (with recent experience in last 12
months) - (REQUIRED)
•
Life support certifications must be from AHA, ARC, or ASHI
- (REQUIRED)
•
No hits on state license to practice - (REQUIRED) (DOCUMENT)
•
RTO cannot exceed 7 days, No RTO during the first week of
assignment - (REQUIRED)
•
Candidate driver's license to verify residency - (REQUIRED)
•
Travel experience - (HIGHLY PREFERRED)
