Providing telephonic care and support, the full-time Telephonic Nurse Practitioner - CO Licensed will manage patient care coordination and deliver quality healthcare services remotely while fulfilling a commitment of 40 hours per week, including evenings, nights, and rotating weekends. Key responsibilities: Deliver telephonic and virtual care to patients, coordinating with caregivers and healthcare facilities Conduct comprehensive assessments and document findings in compliance with regulations and quality standards Coordinate care transitions and adjust care plans based on patient needs and responses to treatment Required qualifications: Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program Active and unrestricted Nurse Practitioner license in Colorado Current Nurse Practitioner certification through a national board in a relevant specialty Proficient in using electronic medical records for documentation Ability to work scheduled shifts, including weekends and holidays
