Telephonic Care Navigator

Working remotely in Washington or Oregon, the full-time Telephonic Care Navigator will complete telephonic outreach to members, educate them on care gaps, and assist with accessing community resources while maintaining high performance metrics. Key responsibilities: Completes telephonic outreach to members using various call methods, including auto-dialer and manual calls Educates members on care gaps and helps schedule provider appointments to close those gaps Links members to community resources addressing social determinants of health Required qualifications: High School Diploma / GED or equivalent work experience 1+ years of call center and/or telephonic customer service experience 1+ years of professional experience in an office setting using telephone and computer 1+ years of Healthcare/insurance experience or social work/community outreach experience Ability to work full-time, Monday - Friday, with flexibility for 8-hour shifts
