Unit Details: The telemetry unit serves a very sick, high-acuity population, including patients with pneumonia, end-stage renal disease, post-cath-lab patients, respiratory distress, CHF, and COPD. Common diagnoses at St. John’s Pleasant Valley include postsurgical, orthopedic, and cardiac cases. St. John’s Pleasant Valley’s ACU combines telemetry, medical-surgical, and DOU care; chronic ventilators may be present in the DOU, but travelers will not work there. At St. John’s Pleasant Valley, ratios are 1:5 for medical-surgical, 1:4 for telemetry, and 1:3 for DOU. Telemetry uses Phillips monitors with a monitor technician on each unit; nurses read and sign off on strips. Common drips include amiodarone, Cardizem, and heparin, and they are non-titrating. Patients may require BiPAP or CPAP. The assignment involves a lot of primary care nursing. Tech support may be limited because techs may be pulled to sit, and resources are very limited. A hospitalist or intensivist is in-house; hospitalist coverage is shared between the two facilities and is on call after hours. Respiratory therapy and pharmacy are available 24/7, and there is no IV team.
