Telehealth Nurse

To manage symptom-based encounters with patients, the full-time Indiana Licensed Telehealth Nurse will triage calls and video consultations, utilizing evidence-based protocols to provide care recommendations and promote self-care, with the opportunity to work remotely after training. Key responsibilities Triages patient calls by assessing symptoms and determining appropriate care actions Provides health education and navigates patients through the healthcare system Completes Transitional Care Management encounters for post-hospital patients Required qualifications Current, active RN license with a compact license or application for Nurse Licensure Compact Graduate of a School of Nursing, BSN preferred Minimum of five years of experience as a registered nurse, with preferred experience in emergency department or telephone triage Proficient in using electronic medical records and computer applications Strong assessment skills and excellent clinical judgment
