OVERVIEW
We are currently seeking a skilled and compassionate Telehealth Nurse Practitioner licensed in multiple U.S. states to join our growing virtual care team. This role plays a vital part in providing services such as medication therapy management, chronic care support, and care coordination in close collaboration with partnering health centers.
RESPONSIBILITIES
- Conduct virtual assessments and consultations, reviewing patients’ medical histories, medication regimens, and current health status.
- Provide patient counseling and education, delivering guidance on preventive care, chronic disease management, lifestyle modifications, and medication adherence.
- Coordinate care and make referrals by working with health center care teams, specialists, and community-based providers to ensure seamless coordination of care, including necessary referrals and follow-ups.
- Maintain documentation by accurately documenting patient encounters and care plans in the electronic medical record system in a timely and compliant manner.
REQUIREMENTS
- Proficiency with EMR systems and virtual care platforms.
- Background in primary care, chronic care, and medication reviews.
- Strong communication skills and ability to work independently and within a collaborative care team.
- Flexibility to provide care outside standard business hours occasionally.
