Job Description
The Team Nurse RN reports directly to the Practice Manager and Senior Director of Nursing. The role involves performing RN duties in an outpatient primary care or family medicine practice and serving as a clinical resource for the medical practice. The Team Nurse provides high‑quality, direct patient care within a team‑based model to our BIDHC patients and supports the safety, effectiveness, and efficiency of patient care through the delegation of clinical and administrative functions to the Medical Assistant (MA) staff.
Primary Responsibilities
- Coordinate and facilitate quality clinical management of a broad outpatient population, collaborating with providers on preventive care, chronic disease management, and care coordination.
- Collaborate with community‑based providers, follow up on ED visits and inpatient discharges, and assist providers with exams, treatments, and procedures as needed.
- Provide skilled telephonic triage and assessment, education, and coordination of healthcare and community resources.
- Perform direct patient care, including Annual Wellness Visits (AWVs) and Chronic Care Management (CCM).
- Manage INR for patients on Coumadin using a standardized protocol.
- Administer vaccines and medications by parenteral, oral, rectal, or nasal routes under provider orders and company guidelines, providing appropriate patient education.
- Perform nursing activities such as wound care, point‑of‑care testing, documentation in the EHR, scheduling tests and referrals, and monitoring abnormal results.
- Oversee inventory and ordering of clinical supplies and equipment; ensure safe collection, storage, and processing of biologic specimens.
- Provide oversight and delegation to unlicensed staff (MAs, CNAs, Nursing Students) and deliver ongoing education and training.
- Maintain compliance with mandatory annual training and ensure PHI is used only for treatment, payment, and operations.
- Report adverse patient events and near misses promptly to the BIDHC Risk Management Department.
Required Qualifications
- Graduate from an accredited nursing program (Bachelor’s preferred).
- Registered Nurse license in Massachusetts; Basic Life Support certificate required.
- 1‑3 years related work experience required; new graduate RNs considered.
- CPR certification.
- Strong interpersonal, patient communication, and patient education skills.
- Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications.

Age‑Based Competencies
Competent to provide care to the following age groups: Young adults (16‑30), Middle age (30‑60), Elderly (60+).
Social/Environmental Requirements
- Periods of close, uninterrupted attention up to 4 hours.
- Constant response to changing circumstances.
- Potential exposure to adverse environmental conditions and infectious diseases/bloodborne pathogens.
- Daily use of protective equipment (respirator, earplugs, mask, gloves, eyewear).
Physical Requirements
- Medium work: lift up to 50 lb occasionally, 20 lb frequently; stand/move for most of shift.
- Requires standing, walking, sitting, frequenting, bending, fine manipulation, reaching, and occasional wheelchair assistance or patient repositioning.
Pay Range
$32.24 – $55.32 per hour (base hourly wage). Actual compensation may include shift differentials, call pay, premiums, overtime, and other applicable pay practices.
