Job Duties:
Coordinates and monitors daily THRIVE program operations and activities.
Provides case management services to program participants, including assessment, individualized planning, progress monitoring, referrals, and reentry preparation.
Provides daytoday guidance and support to case management staff and assists with maintaining consistent program practices.
Reviews case management activities and documentation for accuracy, timeliness, and compliance with established expectations.
Monitors participant progress and identifies barriers requiring additional intervention or support.
Coordinates program activities, special events, career development opportunities, community partnerships, and other THRIVE initiatives.
Assists with onboarding, training, coaching, and development of staff assigned to the program.
Tracks program data, deadlines, participant milestones, and required documentation.
Communicates program needs, concerns, trends, and accomplishments to program leadership.
Works collaboratively with unit administration, Rehabilitation and Reentry staff, community partners, volunteers, and other stakeholders.
Assists leadership with identifying opportunities to strengthen program delivery and improve participant outcomes.
Performs additional duties necessary to support the successful operation of the THRIVE program.
