Job Duties:
This Case Manager V position coordinates and monitors the daily specialized reentry program operations and activities for the Strength Through Restoration Independence Vision and Empowerment (STRIVE) and Trust Healing Restoration Independence Vision and Empowerment (THRIVE) programs. The position will report to both site locations, O¿Daniel Unit in Gatesville and Bartlett Unit in Bartlett, routinely with headquarters determined upon applicant selection. This position serves as a team lead, providing program and reentry case management services to program participants, including assessment, individualized planning, progress monitoring, resource referrals, and reentry preparation such as employment services, identification document processing and release coordination. The Case Manager V provides daytoday support to fellow case management staff and assists in providing guidance and maintaining consistent program practices. The position is a part of the program intervention team and reviews activities and documentation for accuracy, timeliness, and compliance with established expectations. Additionally, The Case Manger V assists the Program Supervisor in monitoring participant progress and identification of barriers requiring additional intervention or support.
As a team lead, the Case Manager V facilitates many of the activities with onboarding, direct observation periods, coaching and staff development, and proposing or developing training for staff assigned to the program. The Case Manager V is responsible for communicating program needs, concerns, trends, and accomplishments to leadership and assists in identifying opportunities to strengthen program delivery and improve participant outcomes. THE Case Manager V works collaboratively with unit administration, Rehabilitation and Reentry staff, community partners, volunteers, and other stakeholders to ensure the successful operation of the STRIVE and THRIVE programs.

You can balance individual participant needs with larger program goals, and you¿re someone who identifies gaps, develops solutions, and follows them through to completion. You build productive relationships with employers, community organizations, unit staff, and other stakeholders, and you are proficient in Microsoft Office 365 for documentation, data management, and communication. You contribute ideas that support program growth and improvement, and you¿re seeking a role that blends case management, leadership, program development, and meaningful reentry work¿making you a strong fit for this position.
