Job Duties:
Provides individualized case management and reentry planning to THRIVE participants.
Assists participants in identifying and securing housing, employment, transportation, identification documents, benefits, community resources, and other essential reentry needs.
Works with participants to identify barriers to successful reentry and develops realistic plans to address those barriers prior to release.
Assists with employment readiness, including resumes, job searches, applications, interview preparation, and connections with potential employers.
Researches and maintains knowledge of available community resources and develops connections with organizations that can assist participants after release.
Coordinates referrals and follows up to ensure participants are connected with appropriate services and resources.
Meets regularly with participants to monitor progress, update goals, and address changing needs.
Maintains accurate and timely case management documentation and tracks participant milestones, referrals, and outcomes.
Supports THRIVE programming, workshops, career expos, special events, and other participant activities.
Collaborates with program staff, community partners, employers, volunteers, and other stakeholders to expand opportunities and resources available to participants.
Engages in ongoing crossdivision communication and coordination with internal departments.
Communicates barriers, concerns, and participant needs in a timely manner and works with both the THRIVE team and crossdivision partners to identify solutions.
Serves as a liaison between THRIVE and other divisions to ensure participants receive consistent information, coordinated support, and uninterrupted access to services during all phases of reentry planning.
Performs additional duties necessary to support participants and the successful operation of THRIVE.

You can help someone take a large challenge, such as finding housing or employment and turn it into realistic, achievable steps.
You understand the importance of followthrough and don't consider a referral complete simply because a phone number or resource was provided.
You can build positive professional relationships while maintaining appropriate boundaries and expectations.
You are interested in rehabilitation, reentry, and helping individuals successfully transition back into their communities.
You'd be a GREAT fit if:
You have experience in case management, reentry, workforce development, housing services, social services, community resources, or correctional programming.
You have experience connecting individuals with housing, employment, benefits, transportation, identification, or other communitybased resources.
You are comfortable building relationships with employers, housing providers, community organizations, internal departments, and service providers.
You understand that successful reentry requires addressing practical barriers, not just completing program requirements.
You are persistent and willing to explore multiple options when the first resource or plan does not work.
You take initiative, communicate concerns early, and follow tasks through to completion.
You are comfortable working in a collaborative, crossdivision environment where coordination and communication are essential to participant success.
You can work independently while contributing to a team focused on participant success.
You want to do work where you can directly help someone leave incarceration with a plan, resources, connections, and a realistic path forward.
