Surgical Care Navigator

Supporting patient engagement for orthopedic patients, the full-time Surgical Care Navigator will conduct outreach, administer assessments, and coordinate care while working remotely. Key responsibilities Conduct outreach to potential surgical patients and administer assessments to identify care goals and barriers Act as an advocate and educator for patients, assisting with appointment scheduling, transportation, and care coordination Serve as a liaison among patients, providers, and internal teams to promote evidence-based orthopedic care Required qualifications Minimum 3 years of clinical experience in MSK, orthopedic, or value-based care preferred Experience with electronic health records and call center systems Ability to maintain HIPAA and security guidelines while handling protected health information Experience in fast-paced environments or startups Bilingual (Spanish/English) is a plus
